I've thought of half a dozen interpretations of this movie so far, and there are doubtless many more based on our own beliefs and experiences. Truly a brilliant film.

For me, the reveal at the end just proved that not only were Hell and demons literally real but that God was as well - and every bit as much a game playing creep as described in the Old Testament. Bill Paxton's character was not crazy, he was a tragic hero put into an impossible position through his loyalty to a sadistic overlord who just didn't care about him.
