Oh, PTSD flashback. Good call. The clothes he wears in the first panel are the same as when he's looking in the mirror on the cover, suggesting that's what he's still doing while reliving what happened up to that point.

Note the visual 'shockwave' effect of the gunshot sound in the background, along with the still-present muzzle flash.
