Like youness said, the tape helps it make sense, at least part of it. Johan became confused and mixed up his real memories with the horrors he only heard about, when he started undergoing the brain-altering experiments at kinderheim. In the tape he says his greatest fear is forgetting about Anna, since the experiments are distorting/erasing his memories and sense or self, which was already extremely warped to begin with. At least that's how I always understood it, I could be way off the mark haha. I didnt actually consider that he might have initially tried to shoulder those memories to help Nina, but that's a super interesting perspective that definitely makes sense.
