The main point being that she's off the train, and not getting back on to do anything. She's effectively out of the story, even if she lucks out, gets help, and survives.

That raises a good question about the tech level of this world: it's generally behind ours, but specifically what do they know about antibiotics? A penetrating wound of the abdomen is bad news for us, but with antibiotics to treat the peritonitis it's not a death sentence. Before penicillin, before sulfa, it pretty much was. In a world where saving a life is less important, and a dead patient is just food for other patients, maybe they don't have the drive to research how to treat infection.

One of the many problems that come from jumping out of a moving vehicle is the fact that you don't know what you'll be landing on. Could just have easily fallen on some broken bottle, a really big rock, a really sharp rock, etc.
