There's a Vanilly Cake, which Ava seems to enjoy eating. There's a drawer and a door. And judging by the previous panels it seems like Ava might be transitioning into a flashback to explain why the drawer being on her chest would be "spiteful". I'd bet a 70% chance on a flashback.
