Bitsalina can kick things off after noticing there's something wrong, but you could also have a bystander freak out and then suffer a painful, grisly death that compels the party to protect others as the place turns into a massacre. If you want to draw it out a bit instead, gradually make the subtler influence of the fiends (smell of brimstone, strange shadows, an aggressive energy roiling in the air) more and more noticeable while dropping more hints from conversation about the cultists' true (evil) motives. Then when it's time to draw swords if the party are hesitant have Bitsalina (or the fiends!) make the first move.

I'm having a hard time justifying the breakout of hostilities. It seems like it's a little rushed. Any thoughts on how to roleplay it more?
