May '24 Rewards Form

Had a college finals and a emergency room visit, it was a SVT episode but am mostly fine now and going to see a cardiologist to see if there's any concern. (Baby is fine though ^^).

Because I had several doctor appointments coming up, I decided to take some time off from my job which means I should be able to knock out my April and May rewards in the 2nd half of this month. : )
