I think that's the matter of slider or other item crushing... Can you check with other sim, or remove other items(accessory, change the preset, etc.) and check whether it keeps happen? I have heard some eyelash items conflicts.

Can you check with other sim, or remove other items(accessory, change the preset, etc.) and check whether it keeps happen? If so, I'll re-check the weight paint and vertex map again
