The answer to all your questions but one is the incarnation system, its a system thats in the game that gives you immense power through strong emotions. the first time we saw it was in episode 10 when asuna ran at super human speed to save kirito from kuradeel. the next time we saw it was during the fight with kayaba, when asuna used it to remove her paralasys after that kirito cheated death with it, we know that because he saw Asuna just befor he died and remembered their promise, that he would get her back to the real world. thats why he survived for long enough to kill kayaba thats why he didnt die. because the game ended before kirito died At 13:35 you can see Kirito has golden eyes, thats how the Anime visualises MAX Incarnation, so if you ever see Kirito with golden eyes you know what it means Asunas on the other hand did really die, BUT Kirito made a deal with kayaba before the fight " make it so Asuna cant kill herself if i die" thats what she tried to do to save kirito i hope i answered your questions, i have no idea why the anime didnt cover this system (they talk about it in like 50 episodes)
