Jareth has his own private practicing doctor due to the danger of going to a public one. They arranged the surgery through this private doctor. Top surgery would work the same as it does now, a very similar procedure would be used. However, it would be seen as very experimental and risky, and the results would be a complete mystery (the results would turn out good!)
