So what's the story here, you ask? When did this happen? Remember the BIG EXPLOSION when Baby Faerie was born? (That's around comic #413, for those of you who want to check the backlog.) Big badaboom! That big explosion (whose effects I never really explained in the comics) caused anyone who was fae-touched to be permanently affected by a magical curse. Patience, who'd been the target of several faerie spells, kind of had a mind-blowing climatic moment and got a bonus boy part as well as spectacular (and lactating) boobs (not that they weren't already spectacular to start with).
So this is that moment.
